Keeping track of your Right to Choose referral

Looking for an ADHD assessment can involve exactly the things you already find difficult: forms, emails, remembering dates and following up. If your Right to Choose referral feels like another unfinished project, a small tracking system may help more than a complicated spreadsheet.

Right to Choose applies in England when the relevant NHS patient-choice conditions are met. Discuss the referral with your GP and check the chosen provider’s current instructions. Providers may ask for a particular referral form, screening questionnaire or supporting information. Completing a questionnaire is not a diagnosis, and your GP still needs to consider whether referral is appropriate.

Create one place for everything. That could be a phone note, an email folder or a paper envelope. Record the provider’s name, the date you spoke to your GP, what you have been asked to send and the next action. Keep copies of completed forms where possible, so you do not have to reconstruct your answers later.

Once the practice says the referral has been sent, check how the provider confirms receipt. Do not assume that sending paperwork means you are already on a waiting list. If confirmation does not arrive within the provider’s stated timeframe, contact the practice or provider to ask where things stand.

Set a reminder based on the guidance you receive rather than checking every day. Waiting times can change, and a website estimate is not a promise about your appointment. If your contact details change, tell the provider and your GP practice. Check spam folders occasionally so appointment messages do not disappear unnoticed.

If paperwork stalls, ask someone you trust to sit with you while you complete one step. You can also ask the practice or provider about accessible ways to communicate. For questions about NHS choice rights, use official NHS patient-choice information. Organising the process does not guarantee a faster assessment; it simply makes your own next action easier to see.
